Financial and Progress Report Forms

A financial report form must be submitted annually by the host institution for each research project for which funds have been awarded by Cystic Fibrosis Canada. Patent information and equipment in the range of $10,000 (with prices paid) should be itemized; supplies/expendables need not be itemized.

The financial report form must be completed and returned to Cystic Fibrosis Canada’s office by June 1st. If financial report forms for the granting period ending March 31st have not been submitted by September 1st, the October 1st quarterly payment will be withheld.

Statements for studentship and fellowship awards are not required.

A progress report form is required from Cystic Fibrosis Canada-funded research grantees, senior scientist research training awardees, fellows and students. This report highlights research activities and any publications during the tenure of the grant/award period.

Progress report forms must be completed and returned to Cystic Fibrosis Canada’s office no later than three months following the grant/award end date.
