CALL FOR COMMUNITY STAKEHOLDER APPLICATIONS
Cystic Fibrosis Canada is Seeking a Community Stakeholder Representative to Join the Scientific Review Panel
Cystic Fibrosis Canada invites you to apply to participate, as a community stakeholder representative, on our organization’s scientific review panel. This is an opportunity for informed members of the cystic fibrosis (CF) community to play a vital role in the review process by which funding is allocated. Community stakeholders serve as a sounding board, and as a source of advice, with respect to the identification of research proposals that have high relevance to CF.
This year, Cystic Fibrosis Canada is looking to select one individual living with CF to serve as a community stakeholder member on the scientific review panel. Please see the Terms of Reference for further details on the role.
Note, there will be a mandatory, formal training session for community stakeholder members via WebEx video conferencing (date to be determined).
Application deadline: Friday, September 6, 2019.
Incomplete and/or late applications will not be considered.
Applicants must submit a completed Community Stakeholder Member Application form and supporting documentation via mail to email@example.com.
The following information must be completed and included in the application package:
- completed application form
- current résumé which includes your current or prior experience serving on a Cystic Fibrosis Canada committee or Cystic Fibrosis Canada Chapter/Association Executive committee (maximum 2 pages);
- a completed Report Form for Community Stakeholder Members (page 2 of the application form) to record comments on the relevance of the sample project summary (written in lay language) of CF research (page 3 of the application form);
- a confidential letter of support from a current member of the organization’s Board of Directors or from a Chapter/Association President or Vice President
Please direct questions to firstname.lastname@example.org.
